Performs nursing/health care service to all Supervised Group Living (SGL) residents receiving services from Client Support Services (CSS) in a variety of settings. Participates in multi-disciplinary treatment team approach.
Essential Duties/Responsibilities |

- Maintains primary responsibility for all health care needs of the residents of the group homes
- Conducts health assessment on SGL residents, as needed, links client with needed medical services, monitors follow-up
- Insures that all laboratory information ordered by HCI psychiatrists related to diagnosis and medication is in the client’s chart at the Main Center and updates as needed
- Participates in multi-disciplinary treatment team staffing for the group homes
- Assists in insuring that all group home residents have annual TB screenings
- Assists with educating clients regarding the need for medication and possible side effects
- Assists in conducting health care training for group home staff as well as CSS staff; monitors skills for group home technicians
- Conducts psycho-educational groups for clients and family members/significant others related to the issues surrounding serious and persistent mental illness
- Insures that all physician orders are obtained and transcribed correctly
- Conducts quarterly assessments monitoring for side effects of medications
- Assists in providing on-call coverage for CSS population
- Assists in ordering all medications and pharmaceutical supplies for group home
- Maintains consistent, timely and accurate documentation including client and program records and statistics
- Provides case management services including monitoring, linkage, treatment and service planning for clients
- Utilizes personal transportation in provision of services including transport of clients and in other work-related responsibilities
- Maintains consistent professional and ethical standards in performance of all work-related activities
- Maintains a positive, flexible, and creative attitude, focusing on a strength-based, success-oriented philosophy in working with SMI clients
- Develops and maintains knowledge and technical skills in all areas of service delivery for SMI clients
- Performs other duties as assigned
Minimum Qualifications/Requirements |
- Associates degree in nursing
- Experience in delivery of community based services to target populations preferred
- Willingness to travel and use personal transportation in work capacity
Certificates, Licenses, Registrations |
- Maintain a valid License in State of Indiana as a Registered Nurse
- Maintain a valid Indiana Chauffeur’s license in good standing
- Maintain TB Screening Certification
- Maintain current Crisis Prevention Intervention (CPR) certification
- Maintain current American Red Cross CPR/1st Aid certification
- Lift up to 25 pounds.
- Tolerate temperature variances.
- Drive to other locations, in and out of car many times during work day.
- Adapt to stressful situations.
- Services are provided in community-based settings. Conditions may vary widely.
- Satisfactory reference and background investigation checks
- Completion of tuberculin screening no later than three days prior to the new employee’s first work day and annually thereafter
- Completion of a pre-employment drug screening and completion of post-employment drug or alcohol tests upon reasonable suspicion of use
- Completion of Center-wide new employee orientation and ALL required paperwork prior to reporting to work
- Completion of HCI’s Quality training
- Attendance at all mandatory staff development and training
- Successful completion of a six month on-the-job orientation period
- Participation in payroll electronic deposit
- Adherence to Compliance Program Plan
- Satisfactory completion of an Indiana Department of Child Services criminal fingerprinting background check at the time of hire and every four years (or at contract renewal when applicable) for any employee that has direct contact on a regular and continuing basis with DCS and IVB clients
